If you or a family member has ever been seriously ill, then you know how difficult and overwhelming it can be to coordinate all the doctors and care options available. What is care management and how can it help? Care management helps overwhelmed patients and families figure out the best course of treatment. Nurses, doctors, family members, and the patient all work together to make sure medications are correct and do not overlap or have adverse reactions with one another. One doctor will know the treatment opinions of other doctors and have the opportunity to either alter his course of treatment to allow for other doctors, or share his opinions of why other treatments should be changed. Patients no longer have to carry test results from one office to another or have to make sure they keep lists of medications up to date. All of this information is centralized and distributed to physicians, which makes patient care less frustrating for everyone involved.
